How do I schedule an appointment?
Since you’re already on our website, you can request an appointment by clicking the link on the right hand side of this page. The page will link you to a form for you to indicate your general availability. Please be advised that our providers do book up quickly. If you prefer to call us directly, you can reach our front desk at (949) 831-3057, Mon-Thu 9:00am-5:30 pm.
The forms needed for your appointment can be downloaded using the link on the right hand side of this page. It is best if these are completed beforehand. If not, please arrive at least 15 minutes prior to your appointment to complete the forms. Please bring your insurance card and photo ID to your appointment.
What type of insurance plans do you accept?
We accept most PPO plans and we also accept POS/Open Access plans where you are able to choose your own dermatologist. Below is a partial list of the plans which we accept. If you don't see your plan listed, you may contact your office and we will try to help you determine if your visit will be covered by your insurance. Optimally, please contact your insurance carrier to make sure that we are covered by your individual insurance plan. Their number should be listed on the back of your insurance card. We do accept patients that are uninusured or out-of-network, but the patient is expected to pay at the time of service.
